CONSENT AGREEMENT

I certify that I am fully aware that sunless tanning process is not an exact science and thus relieve La Veronique Georgetown Salon and hold them harmless from all liability for any skin reaction, side effects or damage that my occur to me. I am aware that although the airbrush tanning solution La Veronique Georgetown uses is FDA approved for use as a self tanning skin agent, the FDA has not tested the DHA based products on areas such as the eyes lips, and nasal passages. I have been provided with spray tan care instructions, which I have read and understand completely. To my knowledge, I have no medical condition or allergy which would preclude me from having this procedure done. I have been honest and accurate about the information that I have provided on this waiver. I take sole responsibility of any reaction I may have, staining of clothing and/ or personal belongings. I have disclosed all relevant medical conditions or concerns I have with La Veronique Georgetown salon staff.
